ADMISSION REQUIREMENTS

PHCA is fully accredited by the Florida Association of Christians Colleges and Schools (FACCS) and the International Association of Colleges and Christian Schools (IACCS).  FACCS is a member organization of the National Council for Private School Accreditation (NCPSA).

NEW STUDENT

 K3 – K5

  • Application for Admission – Completed along with the following documents
    • Florida School Physical
    • Florida Certificate of Immunization
    • Copy of Birth Certificate
    • Copy of child’s Social Security Card
    • Legal Documents, if applicable (Injunctions, Power of Attorney, Guardianship, etc.)
  • Pre-Registration Policy Acceptance Form
  • Recommendation Form for Pre-School and Kindergarten completed
  • Interview and Tour completed
  • Registration fee

 

1st – 5th GRADE

  • Application for Admission – Completed along with the following documents
    • Florida School Physical
    • Florida Certificate of Immunization
    • Copy of Birth Certificate
    • Copy of child’s Social Security Card
    • Legal Documents, if applicable (Injunctions, Power of Attorney, Guardianship, etc.)
  • Pre-Registration Policy Acceptance Form
  • Copy of most recent/current year’s report card and/or progress report and prior year’s final report card
  • Copy of the most recent standardized test (i.e., FCAT, SAT, etc.)
  • Interview and Tour completed
  • Registration fee

 

6th – 12th GRADE

  • Application for Admission – Completed along with the following documents
    • Florida School Physical
    • Florida Certificate of Immunization
    • Copy of Birth Certificate
    • Copy of child’s Social Security Card
    • Legal Documents, if applicable (Injunctions, Power of Attorney, Guardianship, etc.)
  • Pre-Registration Policy Acceptance Form
  • Copy of most recent/current year’s report card and/or progress report
  • High School Transcript reflecting all graded earned in the 9th grade and above.
  • Two Character Reference (Teacher and Non-family Member)
  • Discipline Reference completed by previous/current school administrator
  • Copy of the most recent standardized test (i.e., FCAT, SAT, etc.)
  • Request for Records
  • Interview and Tour completed
  • Registration fee

 

RETURNING STUDENTS

Thank you for your continued support and your desire to re-enroll your child at The Potter’s House Christian Academy.  Every parent must complete a re-registration packet in order to enroll his/her child for the upcoming school year.
